First Steps Maternity Support Services (MSS) and Infant Case Management (ICM)

"First Steps MSS services can begin any time during prenatal, delivery or postpartum period up to 60 days following the end of pregnancy. ICM services can start after MSS ends (when baby is about 3 months old) and can continue up to the last day of the month when baby turns one.

Mom and toddler
Services may be provided in an office, home, or alternative location.
Services can include:
Assessment
Education
Referrals to resources in community
Interventions
Brief Counseling
Services are provided by a multidisciplinary team.
Each team includes:
Nurses
Nutritionists
Behavioral Health Specialists
Community Health Workers"
